Preparing a written statement checklist

  1. Have you stated your business name and address (registered address if a company) and the employee’s name and address?
  2. Have you confirmed the employee’s start date and the date on which continuous employment began? These two dates are not always the same (see our guidance notes on Contracts of Employment).
  3. Have you included the job title or brief description of the work? Consider attaching a copy of the employee’s job description as an appendix.
  4. Does the statement confirm the place of work or various places of work (as appropriate)?
