  1. Has the employee notified you in writing of her pregnancy?
  2. Have you carried out a risk assessment and taken any necessary action?
  3. Has the employee notified you of the expected week of childbirth (EWC) and of the intended start date of her maternity leave, at least 14 weeks before the EWC, or as soon as reasonably practicable?
