Parental leave checklist

  1. Does the employee’s request, oral or written, specify the date on which leave is to start and end?
  2. Has at least 21 days notice been given?
  3. Has the individual got at least one year’s service by the date leave is due to start?
  4. Is the employee eligible to take leave in respect of this child?  Do you require evidence to this effect or is this evidence already contained on the personnel file?
