Time off for dependants checklist

  1. Is the individual an employee?
  2. Is the person in respect of whom the employee wishes to take leave, a dependant as defined by the regulations or your own company policy?
  3. Has the employee given the reason for the leave and indicated how long they will be absent? If not, is there a reason for this?
  4. Is the reason for which the employee wishes to take leave covered by the regulations or specified within the company policy?
